OptimumCare4U provides CQC-registered community supported living for adults with a forensic mental health history, including individuals on Section 37/41 restrictions, Community Treatment Orders (CTOs), and MAPPA Levels 1–3. We are experienced in sourcing accommodation that meets licence conditions and victim safety exclusion zones, and in managing complex risk in community settings.

Every forensic placement is individually designed. We do not place individuals into existing shared houses. We source bespoke accommodation against each person’s clinical needs and risk profile, staff the placement individually, and co-produce a risk management plan with the responsible clinician, MAPPA coordinator, and placing commissioner before the person moves in.
We use the HCR-20 structured professional judgement tool for dynamic risk tracking, reported quarterly to commissioners alongside HoNOS and CORE-10 outcomes data.
Following NHS England’s direction to remove patients from St Andrew’s Healthcare Northampton, we have placement capacity available now. We have designed a bespoke trauma-informed transition pathway for this cohort, including named keyworker introduction before move-in, graduated community familiarisation, and open communication with the responsible clinical team.
